LCSW / Behavioral Health Specialist

Location: Seattle, WA

Pay: $58,000 - $85,000/yr + Sign-On Bonus

About the Role

As a Behavioral Health Specialist (LCSW), you will support older adults enrolled in a comprehensive healthy aging and wellness program. This full-time, community-based role blends clinical social work, behavioral health counseling, and care coordination to help participants age safely and independently while managing complex medical, behavioral, and social needs.

Key Responsibilities

• Conduct in-person psychosocial assessments and provide ongoing therapy, counseling, and crisis intervention.
• Collaborate with an interdisciplinary care team to develop and manage individualized care plans and transitions of care.
• Perform home and community visits to assess safety, supports, and living environments.
• Advocate for participants and assist with benefits, housing stability, advance care planning, and community resources.

Qualifications

• Master’s degree in Social Work and active Washington State LICSW licensure.
• Minimum of two years of behavioral health or clinical social work experience.
• Experience working with older adults, medically complex, or vulnerable populations preferred.
• Valid Washington State driver’s license with ability to travel locally.
